Step 1: Complete Client Interest Form
If your interested in having one of our staff members contact you for a one-on-one appointment, please complete our Client Interest Form. Once we've reviewed your information, we will contact you via email to schedule an appointment. We know your time is valuable and that you may be researching other Employment Networks. We will do our best to get back with you right away so you can move forward in the process.
Please note that Ticket assignments are reviewed and accepted on a case-by-case basis. The Client Interest Form is a way for us to begin the evaluation process and assess the individual needs of each Ticket holder. It is not a guarantee of enrollment into the program

Step 2: Develop Individualized Work Plan
If we agree to move forward with the enrollment process, the next step is to develop your Individualized Work Plan (IWP), which will include your career or educational goal and the supports and services you may need to reach it. We'll develop this plan together and mail it to you for your signature and approval. Once we receive your signed IWP by mail or fax, you will receive an acknowledgement receipt by e-mail. Your IWP will be then processed within five (5) working days. We'll then mail a signed copy of the IWP back to you.
In the meantime, we'll send you a weblink to some additional program documents for signature and also e-mail you the necessary information to access the online portion of our services. Please make sure you've added us to your “safe” list so you will receive the e-mail without delay. At this time, you can begin accessing the services and supports available through our Ticket to Work program.
